There's a website called Response-o-matic.com (I think) that will email the form to you whenever someone hits "submit". From what I've read in a "Build Your Site the Right Way using XHTML and CSS" book, dealing with forms involves some PHP coding, so on my radio-show site's feedback/question/request form, I just used Response o matic to build the form and set it up so I can get the results as an email.

Hope that helps.
